I know the Heavy Ball will always fail if the Pokémon weighs less than 100kg and has a certain natural catch rate. I know in Gen 1 the Great Ball has a higher catch rate than the Ultra Ball. I’m trying to think of which one has had repeat issues…
Those were the same two programming oversights I was thinking of. If it isn’t one of those two, I’m stumped

Only other thought is fast ball only working on 3 Pokémon it’s intended to in gen 2 or love ball needing it to be both same gender and species in gen 2 and not different gender, but neither are too out there.
There’s lots of balls with isolated oversights but I am not sure what would qualify for the “most”.

No need for interrogation tactics! Stagecoach got it right! The answer is Heavy Ball. It has had oversights in Crystal, Heartgold and Soulsilver, and Sun and Moon. These were all unique as well, and not repeat errors. It’s very obscure and not talked about much so I knew it would stump people. The Heartgold and Soulsilver error wasn’t even known about until a few months ago.
Does mashing A actually help catch rate?
