There are two different Game Boy Color insert cards:

Here is the gen 2 one I’m familiar with:

Does anyone know where that insert card is from? I have the feeling it might not be from a sealed promo, but I haven’t been able to figure out where it’s from. I hope anyone here perhaps knows.
