I bought this card several years ago and just stored it in my collection. I didn’t pay much attention at the time but the PSA label is really rippled and some of the silver font on the back has chipped and left glitter specs. I know some older slabs are prone to that label ripple, but not really to this degree. Does anyone know what causes this to happen? 
Probably humidity and/or heat

I’ve read about this happening to slabs stored in safes.

As long I’ve collected graded cards aka 7 years this has been issue for freshly graded PSA cards. I have many cards in my collection from own middleman submissions + cards bought directly after seller got them back and they haven’t gone any worse with proper storing methods.
