I can confirm that the Japanese EX Ruby & Sapphire set has multiple codes as well. Can’t say for sure about the 1st editions though since I don’t have many 1st editions of that set. I would assume all EX series have multiple codes. I will check to see if I saved any code variants of the other sets as well later, but here’s confirmation for ex ruby & sapphire:
Also, as for the no rarity/rarity cards with the gym sets, only the cards available in theme decks lack a rarity symbol. Cards from the booster packs all have rarity symbols. The theme deck cards are partially exclusive, many cards in the theme decks do not appear in the booster packs and vice versa many cards from the booster packs don’t appear in the theme decks. You might want to differentiate between these sets (theme deck / gym 1 / gym 2) for clarity’s sake.
For the other wotc era sets besides base, it’s pointless to mention “with rarity symbol” cause they all come with rarity symbol. I think all fixed theme decks come without rarity symbol, but the only other set I can think of that can come with and without rarity symbol is the quick starter gift pack set (no rarity) in contrast to the vending series cards (with rarity).
I actually came to that conclusion this Saturday as well. I’m currently reorganizing my binders, and I order the Pokémon on National Index → Set release date → Language → Set number → Specials (stamps, error, etc.)

It probably misses some Decks / releases, since I’ve only made it last weekend, but it should cover most of it. Will edit the main post of this thread later and indeed remove the No rarity. Perhaps I’m able to find information on the release languages for all the Decks and such, although I doubt it (and for promos / special releases it’s a complete mix on which country did or did not release it.
Yep, that’s indeed correct. And only the non-holo Commons, Uncommons, and Energy cards from sets have multiple codes. Promos, stamped cards, and cards from decks will have codes as well, but they’ll all be the same for those cards, so you can basically ignore them.

The only exception are the EX Emerald non-holo Uncommons. For some reason, there are only single codes for these in English, but two each in French, German, Italian, and Portuguese (the second non-Holo code is the same as the Reverse Holofoil code for these).
Japanese sets that have codes will have four for Commons, instead of three like the English ones. Japanese Uncommons will still be two codes.
Holofoil Rares, Reverse Holos, and non-Holo Rares will always have a single code regardless of the language.
And you’re indeed correct that there aren’t any codes outside of the ex era.

My single Pokémon lists do indeed include the English World Championship Deck cards. Unfortunately World Championship Deck cards and misprinted cards aren’t mentioned on the Bulbapedia pages themselves, so those are a manual check afterwards when I create my lists.

@joponnes, Code variants are not difficult to find at all, especially for common cards! Talking from personal experience as these variants were my goal for my Magikarps this past month. And it’s also relatively cheap
Yeah, it looks like the first person i asked had all the variants i was missing so luckily not that big search i really have enough big searching projects atm so didn’t really need another one
Bump. Fixed some things in the first post that I added back in 2017, and updated it with all sets and languages.

EX Battle Boost:a) I’ve seen some Japanese pack openings on YT and sometimes there is no Reverse Holo. Which means that either only a selection of cards of the set have a parallel Reverse Holo version, or either all cards of the set can be Reverse but the Reverse Holo is not guaranteed per pack.b) I’ve seen some Korean pack openings on YT and, likewise, sometimes there is a Reverse Holo.
